For self-employed individuals, who pay Ksh 500 monthly, a Ksh 250 penalty will be charged. In addition to the penalty, it’s a requirement that the premium be paid for the defaulted months. The situation is different for employed individuals as the responsibility to pay by the 9th of every month lies with the employer. Employers can pay the fines through various banks, as directed by NHIF.
Below is a guideline on How to pay NHIF penalty via M-Pesa directly if you’re self-employed.
How to Pay NHIF penalty via M-Pesa
Just like with the monthly premiums, you can pay your penalties via M-Pesa as below:
- Launch your M-Pesa menu.
- Select Lipa Na M-Pesa.
- Click on PayBill.
- Enter the business number, 200222 (NHIF PayBill Number).
- Input your ID number, as the account number, then add letter y after the last number e.g., 24680246y.
- Add the amount of money you’re paying.
- Input your M-Pesa pin.
- Complete the transaction.
- You’ll receive confirmation messages from NHIF and M-Pesa if your transaction is successful.
Below is how to pay NHIF via M-Pesa:
- Navigate to the M-Pesa menu.
- Click Lipa Na M-Pesa.
- Select the PayBill option.
- Enter the Business Number, 200222 (PayBill Number)
- On the account number slot, enter your national ID number.
- Add the amount you’re paying.
- Enter your M-Pesa pin number.
- Click ok to complete the transaction.
If the transaction is successful, you’ll receive a confirmation SMS from M-Pesa. Double check all the details before completing the transaction to avoid sending money to the wrong recipient.
This fund covers Kenyans who are aged above 18 years and anyone working in the formal or informal sectors can apply for NHIF membership. Each applicant should clearly specify that they are either applying as an individual or as an employee.

Outpatient services
- Lab examinations.
- Counselling and wellness services.
- Immunization.
- Physiotherapy services.
- Consultancy services.
Inpatient services
- Surgery.
- Medication.
- Blood transfusion.
- Consultation by a specialist.
Seeing that NHIF is an insurance-related government corporation, you must pay monthly premiums, so that this fund can cover their medical needs in the future. If you work in the informal sector, you’ll be required to pay Ksh 500 per month. The amount to pay if you’re an employee in the formal sector will depend on your monthly income, but it ranges between Ksh 150 and Ksh 1700.

Checking your payment status via phone
You can confirm whether your payment has been received or not and whether you have any penalties by following the simple steps below:
- Launch your Messages app.
- Compose a new message.
- On the message bar, type ID, click on the space tab once, then enter your ID number.
- Send this message to 21101.
On following these steps you’ll receive an SMS, indicating your name, year of birth, ID number, latest contribution, penalties, and your dependents. Note that this SMS costs Ksh 10.
Avoid attracting NHIF fines, by paying your monthly contributions as expected.
How to Select your hospital/facility of choice
Kindly select your preferred hospital/facility using your feature phone (mulika mwizi) by dialing *155#, the “My NHIF” mobile application that is accessible on Google Play/Apple Store, or the self-care portal on our website.
How to Change Out-patient facility/hospital
Facility/hospital selection/change is no longer available in NHIF offices; instead, to select or modify the preferred out-patient hospital, please use the USSD code *155#, or download the mobile app “My NHIF” from the Google Play Store and Apple Store, or visit the website.
How to add dependents
It is not possible to add dependents online. Visit your local National Health Insurance Fund (NHIF) office with a photocopy of your child’s birth certificate and the original (for certification), and for children under the age of six (6) months, a birth notification is acceptable.
How to Choose in-patient Hospital
When it comes to in-patient care, you are not required to use a specific hospital. In-patient services are available at any of the NHIF-accredited hospitals or facilities throughout the country.
How to Update an out-patient facility for a parent, sibling, relative or a friend
The selection procedure is configured to be done by the Member whose mobile phone number is captured in the NHIF system. You can only update the out-patient facility using his/her Mobile telephone number.
